ONCALL is seeking a motivated and compassionate Support Coordinator to join our Queensland team on a full time basis. In this key role you will help our clients with coordinating NDIS plans, negotiating appropriate support, and accessing a range of mainstream services and community support. What you’ll be doing: - Helping NDIS participants to understand their funding.
- Connecting participants with providers, services and community supports.
- Setting up service agreements and monitoring service delivery.
- Tracking service delivery hours and billing accordingly, ensuring that Support Coordination funding is managed effectively.
- Strengthening and enhancing participants’ abilities for greater independence.
- Ensuring all required documentation and reporting is completed as required and in line NDIS Practice Standards.
What you will bring: - A sector relevant qualification, e.g. Bachelor of Social Work/ Psychology, Allied Health, Health Sciences or Community Services.
- Proven experience in a role as a Support Coordinator.
- A track record of supporting participants to access the right supports, to develop and achieve sustainable goals in line with their NDIS plan.
- The expertise to build the capacity of clients and their networks to coordinate their own supports.
- Your excellent assessment and planning skills that enhance outcomes for people with NDIS funded support.
- Excellent communication, organisational and problem solving skills.
