About the Role

We are currently seeking compassionate and experienced Disability Support Workers to join our team in Brisbane's Northern suburbs, with shifts available in Caboolture, Lawnton, Brighton, Stafford, and Boondall.

Supporting our clients requires an empathetic approach, strong relationship-building skills, and the ability to communicate very clearly. You will be supported by an experienced team leader and have access to additional training to manage the complexities of this role.

How Will I Help?

You will empower clients to achieve their goals and provide support in their homes, in community participation or in day centre environments. This will include individual care needs in areas such as wellbeing, housekeeping, personal care, meal preparation, manual handling, administration of medication and daily activities.

What Do I Need?

• Ability to manage 1:1 shifts
• Willingness to work short shifts - ideally multiple shifts in the day
• At least 6 months of local work experience in a similar role in the Disability Sector
• Previous medication administration and manual handling experience is preferred.
• Strong interpersonal and communication skills, both verbal and written
• NDIS Worker Screening Check and current Blue Card (WWCC)
• Drivers Licence, a car and willingness to transport clients

About the Role
You will be supporting a participant with high and complex care needs, requiring consistent 1:1 and 2:1 support, including active and passive overnight shifts.

This role involves delivering person-centred care while supporting safety, dignity, and quality of life across all daily activities.

Key Responsibilities
You will support the participant with:

- All activities of daily living (ADLs), including personal care and hygiene
- Safe mobility and transfers (including 2:1 assist and use of hoist/sling lifter)
- Meal preparation and 1:1 assisted feeding in line with dysphagia guidelines
- Medication support (full assistance required)
- Behavioural support and emotional regulation
- Community access and social engagement where appropriate
- Maintaining a safe environment, particularly managing high falls risk

Complex Support Requirements
This role involves supporting a participant with:

- Neurodegenerative condition (including cognitive and physical decline)
- Dementia-related behaviours and reduced communication capacity
- High falls risk due to impulsivity and unsafe mobility
- Swallowing difficulties (dysphagia) requiring supervised eating/drinking
- Sleep disturbances, including agitation and wakefulness overnight
- Continence support needs
- Behavioural and safety risks requiring structured support approaches
You must be confident working in 2:1 environments, including during transfers, personal care, and overnight support.

Overnight Support
- Combination of active and inactive overnight shifts
- Ongoing monitoring, supervision, and support with mobility and toileting
- Managing sleep disturbances, restlessness, and behavioural responses
- Ability to respond to escalating situations calmly and safely

About the opportunity:

Join our ever-growing team at SACARE as an Intake Coordinator today! We are currently on the lookout for an Intake Coordinator to work either Full Time across our accommodation and community services.

Based at our head office in Broadview, the successful candidate will be responsible for:

• Managing daily enquiries and service requests within appropriate turn-around times and with a person-centred approach.
• Vacancy, pipeline, and waitlist management.
• Daily monitoring of the referrals inbox.
• Collating and gathering relevant documentation from participants and other relevant stakeholders, to determine eligibility to receive SACARE’s services.
• Accurate data management ensuring participant privacy and confidentiality.
• Maintaining accurate records in a Client Relationship Management System (CRM) for weekly reporting purposes.
• Creating and facilitating the development/scheduling of:

       -  Service agreements

       -  Quotations

       - Community visits with potential participants (face-to-face where possible)

       - Initial Support Needs and Risk Assessments

• Collaborating with SACARE’s NDIS Specialist, finance, client services, scheduling and service co-ordination teams, and supported accommodation managers to ensure a timely and effective transition period.
• Participating in various information sessions with the community.
• Attending various internal and external networking events, including exhibiting at expo’s for increased referral sources.
• Stakeholder/referrer relationship management (support coordinators, plan managers, hospital discharge planners, social worker teams, other referrers)
• Collaborating with the Business Development Manager and the Marketing team to develop targeted campaigns, including EDM’s, Social Media posts and networking activities to assist in increasing referrals.

To be considered for the role, candidates need to have:

• Qualifications in Cert IV in Business and/or Community Services (or relevant demonstrated experience).
• NDIS Workers Check and Working with Children Check (Mandatory)
• Excellent administrative, time management and organisational skills.
• Demonstrated experience working within the Disability sector (or Community Services sector), including knowledge of NDIS and DHS standards.
• Understanding of up-to-date Disability related legislation, policy and processes.
• Ability to work autonomously, set own priorities and work to deadlines.
• Ability to work collaboratively and influence a range of internal and external stakeholders, including colleagues, participants, families/carers/guardians.

About the Role

Reporting directly to the Chief Executive Officer and part of the Executive Leadership Team, you will provide strategic and operational leadership across the SACARE | ONCALL Group, ensuring our clinical services, safeguarding frameworks, and quality systems meet legislative, regulatory and best-practice requirements.

You will lead the development and maintenance of robust clinical governance systems, oversee safeguarding and quality initiatives, support innovation in service delivery, and ensure compliance with the NDIS Practice Standards and Quality & Safeguards Framework.

Key Responsibilities

• Clinical Leadership & Governance
• Provide clinical leadership across the organisation.
• Oversee the development and review of care plans for individuals with high and complex health needs.
• Lead clinical governance, policy development, and professional practice standards.
• Provide expert clinical advice and support to operational teams.
• Support onboarding and ongoing oversight of customers with complex clinical needs.
• Lead clinical investigations and incident management processes.

Safeguarding & Quality

• Lead safeguarding strategy, compliance, and continuous improvement initiatives.
• Chair monthly ACRM meetings.
• Develop and review safeguarding policies and procedures.
• Oversee incident management, investigations, and reporting processes.
• Drive quality assurance activities, audits, and service reviews.
• Foster a culture that prioritises safety, quality, dignity, and inclusion.
• Represent ONCALL in external sector forums and industry networks.

Leadership & Improvement

• Lead and mentor multidisciplinary teams.
• Champion innovation and evidence-based practice.
• Build strong stakeholder relationships across the organisation and broader community.
• Support organisational growth and service development opportunities.
• Drive a culture of accountability, collaboration and continuous improvement

You are a strategic and influential leader who combines strong clinical knowledge with exceptional quality and safeguarding expertise. Brings energy and collaboration to quality and safeguarding, ensuring compliance becomes a passion that our teams want to be part of. Works collaboratively across teams to embed quality and safeguarding practices in ways that are practical, engaging, and genuinely valued by our teams, not just another compliance box to tick.

You will bring:

Essential

• Demonstrated leadership experience within the disability sector.
• Experience managing clinical outcomes and governance frameworks.
• Strong knowledge of the NDIS Quality and Safeguards environment.
• Outstanding stakeholder engagement and relationship-management skills.
• Highly developed written and verbal communication skills.
• Strong organisational, analytical and problem-solving abilities.
• Advanced computer literacy, including Microsoft Office applications

Desirable

• Registration with AHPRA and demonstrated maintenance of professional registration requirements.
• Experience leading multidisciplinary teams across multiple service streams.
